Below are the instructions for completing Form 605 MAIN and Form 605 Schedule E .
These forms must be submitted to the FCC along with the original Proof of Passing in order to obtain your FCC license.
Before you attempt to fill out the form you must first obtain a FCC Registeration Number (FRN) if you do not have an FRN please CLICK HERE .
That link will take you to the FCC page for getting your FRN.On that page click on REGISTER and provide the requested information and you will receive your FRN. Once you have the FRN print the page and put it in a safe place. You will be needing it from time to time later.
After completing Form 605 you must submit it, along with your Proof of Passing to:

Instructions:
1. GROL w/ Radar endorsement.
(If you passed Elements 1-3-8 you are eligible to receive the GROL with a Radar endorsment)
You must complete and submit form 605 main and a Form 605 Schedule E.
If you passed Elements 7&9 you are eligible to apply for a GMDSS license. Complete the Schedule E Below and send it along with a seperate form 605 MAIN, PROOF OF PASSING and Schedule E.
